What is My Support Passport?

My Support Passport is a fill-in support guide that helps caregivers and supporters stay consistent with routines, safety needs, sensory supports, communication, and daily care—across home, school, and outings.

How do I choose the right version (Digital vs. Printed vs. Bundle)?

  • Digital: best if you want quick updates + easy sharing
  • Printed: best for a physical copy that lives in your home/bag/support setting
  • Bundle: best if you want both—digital for updates + printed for in-person handoffs

Do I need any special apps or technology?

No. Digital is a fillable PDF (you’ll just need a PDF viewer/editor that supports form fields). Printed requires no tech—write by hand.

How do I access my digital download?

After checkout, you’ll receive an email with your download link (and it may also appear on your order confirmation page).

What do I need to fill out the digital version?

Any PDF reader that supports form fields. Many people use Adobe Acrobat Reader (free) or a built-in PDF app on their device.

Can I use the digital version on my phone or iPad?

Yes—open the PDF in a PDF app/viewer that supports form fields, then tap into the boxes to type.

Can I print the digital version?

Yes. You can print a blank copy to write by hand, or print your completed version after you fill it out.

Can I update it later?

Yes. Update anytime—digital by editing the PDF, printed by writing updates (and using notes pages if included).

Can I share it with others?

Yes—share the completed passport with trusted supporters (family, babysitters, therapists, school staff).

When will my printed passport arrive?

Printed passports are made to order. Please allow 2–3 weeks for production and delivery (U.S. only). Tracking is emailed when your order ships.

Is this medical or legal advice?

What if I have trouble downloading or editing?
